Update all pack READMEs to refer to StackStorm Exchange #649
Description
Some people are still finding their way to this repo.
To help reduce the number of people doing this, we should update all the READMEs. Move the old README to README.old or something similar, and put a new README in each pack directory, pointing to the new pack-specific repo location.
Also update the main frontpage README to only have the top section ("we are moving"), and remove the content below.
