Add monitoring for per-query memory usage

I'd like to see a graph for TiDB that shows per-query memory usage (avg, p90, p99, etc.). I think it would be very helpful to know if there are a lot of queries using very large amounts of memory. This would help us avoid tidb-server OOM.
