Adds Azure OpenAI model definition for gpt-5.3-codex (2026-02-24) with pricing and limits (400k context / 128k output) aligned to Azure documentation, and sets the Aug 31 2025 knowledge cutoff per OpenAI model docs. Also links the model into the Azure Cognitive Services provider and marks GPT-5.1/GPT-5.2 Azure models as supporting structured outputs.
